Position Summary:

Provides coordination and leadership to the Positive Opportunities for Parenting Success program(POPS2) with support from the Family and Community Support Services’ Parent Educator/Advocate Coordinator  and Department Manager. The Lead Case Manager will  oversee coordination of services, which includes but is not limited to working with POPS2 staff and families participating in the program. Will work directly with families to screen for eligibility, develop case plans, and connect families to the resources/services they need. , Supports CAPSLO’s mission by providing direct services to clients and their families to achieve self-sufficiency.

Required Education and Experience:

  1. Bachelor’s Degree in Child Development, Psychology, Counseling, Social Work or Family Studies and three years related experience or equivalent combination of education and/or experience.
  2. Must be bilingual, biliterate, and bicultural in English and Spanish. 

Essential Functions:

Coordinates POPS2 services and works closely with the POPS2 team making sure that services are being provided according to the Supporting Father Involvement curriculum. Works closely with the parents attending the groups and provides case management services. 

Responsibilities and Duties: 

  1. Works with the Parent Educator/Advocate Coordinator to ensure the program goals and objectives of POPS2 are being met as outlined in the Supporting Father Involvement curriculum. 
  2. Assists the Father Engagement Specialist in recruiting families for the program. Develops outreach materials for the program. 
  3. Receives referrals for POPS2 and contacts families to share the goals and objectives of the program and screens them for eligibility. Completes all required program documentation, including the CAPSLO intake.
  4. Schedules the sites for the POPS2 groups, coordinates child care, and dinner.
  5. Attends each group session and makes themselves available for families to talk to regarding any issues/concerns.
  6. Contacts families to remind them about the group sessions and checks-in regarding child care needs.
  7. Actively engages with POPS2 participants, creates a case plan, coordinates a comprehensive and extensive family needs assessment, and works with the family in getting them connected to their needed resources/services.  
  8. Assists and transitions families to community referrals to ensure they are supported (helps identify problems and concerns and links families with appropriate referral systems and services.)  Provides timely follows-up with families to ensure that services are received and encourages families to develop self-sufficiency skills.
  9. Schedules and holds program update and case review meetings with the POPS2 staff to ensure the team is all on the same page regarding the participants and program logistics and status. 
  10. Keeps informed of available resources to families, including community agency services, workshops, and classes. Encourages participation in program and community events.  
  11. Networks with related agencies to link families to POPS2 groups.
  12. As needed, conducts regular home visits with families to help open communication and develop a trusting relationship.  
  13. Collects and compiles descriptive and statistical data relevant to evaluations and goals on a timely basis.  Works with the Parent Educator/Advocate Coordinator and Data Analyst/Program Assistant to complete necessary programmatic reports. 
  14. Develops Board Reports on a monthly basis, updating the Board regarding programmatic services.  
  15. Ensures data is entered timely into the ClientTrack database.
  16. 15. Attends meetings and conferences as required and directed.
  17. Must be dedicated to the goals and philosophy of the division’s child development programs and CAPSLO.
  18. Must use reasonable precautions in the performance of one’s duties and adhere to all applicable safety rules and practices; and act in such a manner as to assure at all times maximum safety to one’s self, fellow employees, clients and children.
  19. Performs other duties as needed.
